
FAith-BASED COMMUNITY PARTNERSHIP PROGRAM
The Faith-based Community Partnership Program (FCPP) is a volunteer-based, community partnership outreach initiative designed to support collaboration between faith-based organizations and communities to provide social service programs. Fostering healthy community partnerships is essential in carrying out the shared responsibility necessary to improve community health and well-being. FCPP strives to maximize faith-based organizations and communities collaboration by:

Establishing Faith-based Community Centers (FCCs) to creatively leverage community resources and services;

Supporting a welcoming and responsive community environment for all families regardless of race, color, national origin, religion, gender identity or expression, sexual orientation, disability, age, socio-economic status, political beliefs, or any of protected affiliations;

Promoting faith-based organizations and community engagement in community health projects and region level decisions;

Increasing access to information to assists community members to navigate resources required to help them become productive citizens;

Responding to community participants’ concerns to ensure community health needs are met;

Providing community participants opportunities to acquire necessary knowledge and skills to build capacity for leadership and advocacy to support overall community health
